CNA-3yrs I worked for New England Pediatric Care as CNA and Teacher's Aide in their Special Education Day School. I assisted mentally and physically fragile children with everyday ADLs (bathing, hygiene, incontinence care, dressing, ambulation assistance and specialized feeding) in the morning then would help transfer them to their assigned classrooms. We would focus on communication skills and individual goals through their IEPs (individual education plan). LPN-4yrs/Present I currently work for Beacon Hospice an Amedysis Company. I provide one on one hospice care to terminally ill patients in their home. I deliver nursing care as outlined in a patient specific care plan for the patient and caregivers. I assess, monitor, document and report progress of patient's health and condition. I spend quality time with the patient to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high quality end-of-life care for the patient.
