About Fawn

Hello there, my name is Fawn and I've been in home health and long term care for the past 5 years. I have been a social worker for 15 years working in the home and community with case management and skilled care. My goal is to keep aging Vermonters in their preferred home setting for as long as safety and medical needs allow. I have noticed that medication administration and meal preparation are the most requested home care tasks by patients and their loved ones. I provide medication administration and case management in the home. I will prepare medications weekly or monthly complete with instructions and consultation as requested. I can provide nutrition consultation in conjunction with meal preparation. I wish to coordinate my services to bridge the lack of caregivers in our rural areas and provide a service that is crucial to keeping seniors in their home while maintaining their independence and autonomy.
